Prior to your first nutrition consultation, please complete this form 1-2 days prior to your scheduled appointment. All services are provided virtually, making it easy for families in Port Perry, Durham, and beyond to access expert nutrition care from the comfort of home.

Electronic Money Transfer to info@sproutandshinenutrition.com or by credit card (prompted when you book). Registered Dietitian visits provided by Sprout & Shine Nutrition may be eligible for coverage under some Extended Health Care Benefits. Sprout & Shine Nutrition services are eligible for private insurance coverage and a detailed receipt is provided following every session which can be submitted for reimbursement. We recommend checking with your insurance provider to see if you or your child are covered!
Cancellation Policy
We require a minimum of 24 hours notice for any cancellations or changes to your appointment. Patients who provide less than 24 hours notice, or miss their appointment, may be charged 50% of the appointment cost, applied to the credit card on file. We accept e-mail cancellations.
